Job Description: ACO Manager
Help Drive the Future of Value Based Care
Value Services Management Group (VSM) is expanding its Traditional Medicare Accountable Care Organization (ACO) portfolio and is seeking an experienced ACO Manager to serve as the operational lead for one or more assigned ACOs.
This is a high visibility role for someone who thrives on relationship management, operational execution, provider engagement, and performance improvement. As the primary point of accountability for your assigned ACOs, you will work directly with providers, practice leaders, and internal teams to translate strategy and performance data into measurable results.
About the Role
The ACO Manager serves as the day-to-day liaison between VSM and participating provider practices. This individual is responsible for driving performance improvement, facilitating provider engagement, coordinating governance activities, and ensuring accountability across all operational initiatives. Every operational thread, performance opportunity, provider concern, and governance matter ultimately rolls up to the ACO Manager.

Key Responsibilities
Provider Relationship Management
• Serve as the primary point of contact for assigned ACO practices
• Build and maintain trusted relationships with providers and practice staff
• Travel onsite periodically to support provider engagement and performance improvement initiatives
Performance Monitoring & Provider Engagement
• Conduct monthly meetings with assigned practices
• Review performance against quality, utilization, and operational goals
• Identify barriers, opportunities, and action plans
• Ensure all action items are documented, assigned, and tracked through completion
Performance Reporting & Analytics
• Deliver and explain monthly ACO performance reports
• Review quality gap reports, Annual Wellness Visit opportunities, utilization trends, and HCC coding performance
• Translate performance data into practical operational recommendations
• Align provider activities with quarterly performance goals
Education & Performance Reviews
• Prepare and present quarterly performance reviews to leadership
• Facilitate quarterly provider education sessions
• Develop educational materials and track participation and follow-up activities
Governance & Committee Support
• Coordinate board and committee meetings for assigned ACOs
• Partner with leadership and Medical Directors to develop agendas and meeting materials
• Document decisions, motions, and governance activities
Qualifications
• 5+ years of experience in value-based care, ACO operations, healthcare operations, or provider relationship management
• Long Term Care experience preferred
• Understanding of quality performance measures, utilization management, and risk adjustment concepts
• Strong ability to interpret healthcare data and communicate actionable recommendations
• Excellent presentation, facilitation, and relationship management skills
• Strong written communication and documentation capabilities
• Experience with CRM, project management platforms, and Microsoft 365
• Ability and willingness to travel to assigned provider practices
